Informed Consent

Before you commence the first session of therapy/coaching, it is important you are aware of and understand the key aspects of the service we are providing. You are free to seek further clarification from MindFi about any of the information contained in this form. We require you to then click ‘Agree’ at the end of this form to proceed with your session.

Services: The MindFi Platform (“Platform”) may be used to connect you with a Therapist/Coach (“Provider”) who will provide services to you through the Platform. We require every Provider providing services on the Platform to be qualified and certified by their respective country’s regulations and professional boards, where applicable, after successfully completing the necessary education, exams, training and practice requirements. The Providers are independent individuals who are neither our employees nor agents nor representatives. The Platform’s role is limited to enabling your interaction with Provider while the Provider themselves are the responsible for the services they provide. If you feel the services provided by the Provider do not fit your needs or expectations, you may change to a different Provider who provides services through the Platform. If a Provider you have been connected with stops using the Platform at any time after you have been connected, we will send an email to notify you that your Provider is no longer on the Platform and that you have the opportunity to match with a new Provider.

Whilst we hope that Provider’s services are beneficial to you, you understand, agree and acknowledge that they may not be the appropriate solution for everyone’s needs and that they may not be appropriate for every particular situation and/or may not be a complete substitute for a face-to-face examination and/or care in every particular situation.

Confidentiality: Therapy/coaching is a private and confidential form of support and advice. All information about our clients is securely protected. We will not give your name or any information about you to anyone else without your signed authority to do so. There are three areas of exceptions where we might have the ethical or legal right to waive confidentiality to involve other relevant parties. The three areas are credible threat to self, credible threat to others, or gravely disabled due to a mental condition. e.g. if we had reason to believe that someone was of significant risk of harm to themselves or others; or to prevent a miscarriage of justice.

Case Notes: Brief case notes will be recorded to assist our Provider to monitor progress towards your therapy/coaching goals.

Cancellations/Changes to Appointments: Whenever possible, please give a minimum of 24 hours’ notice.

Evaluation of Service: On completion of your programme, you will receive a notification to complete a brief survey. Completing the survey is optional and all feedback will remain anonymous. The combined feedback we receive from all helps us monitor the quality of our services.

Complaints: Should you be unhappy with the service at any time or have any comments/questions or complaints, please discuss these with us at help@mindfi.co  

For emergency situations: If you 

  1. are thinking about suicide
  2. are considering harming yourself or others
  3. feel that any other person may be in any danger
  4. have any medical emergency

You must immediately call the emergency service number in your country and notify the relevant authorities. seek immediate in person assistance. The platform is not designed for use in any of the aforementioned cases and the Provider cannot provide the assistance required in any of the aforementioned cases.

The platform is not intended for the provision of clinical diagnosis requiring an in-person evaluation and you should not use it if you need any official documentation or approvals for purposes such as, but not limited to, court-ordered counseling. It is also not intended for any information regarding which drugs or medical treatment may be appropriate for you, and you should disregard any such advice if delivered through the platform.

Do not disregard, avoid, or delay in obtaining in-person care from your doctor or other qualified professional because of the information or advice you received through the platform.
